Team Waivers

To participate on the Swim and Dive teams, waivers must be completed.

• Everyone must complete the Team Waiver

• Complete the Swim and/or Dive Team Waiver if you participate on that team (note if you compete on both, you must complete both waivers)

• Divers must complete the Hand Spotting Form
